The FMIG interest form is for student contact information and opportunity preferences, not clinical information.
What the interest form collects
The form asks for your name, email, optional phone number, class year, selected opportunity interests, and an optional note about what you want FMIG to help you find.
How the information is used
UMiami FMIG uses the submitted information to organize the student interest list and route relevant scholarships, research, travel, volunteering, mentorship, leadership, career, and project opportunities.
Where the form goes
The form submits directly to Zoho CRM using an HTTPS form endpoint. The website does not load Zoho analytics or advertising scripts. The rest of the public site is static and first-party.
Do not submit sensitive clinical information
Do not enter patient names, medical record numbers, protected health information (PHI), passwords, Social Security numbers, financial account information, or other sensitive clinical or identity data in the FMIG form.
